RI 2023 State Plan on Aging Survey

The Rhode Island Office of Healthy Aging is pleased to share with you the 2023 State Plan on Aging Community Survey.

This survey has been created for Rhode Island residents aged 62 and older. Your responses will help inform the Office of Health Aging's efforts to request support and services from the Federal Government through the 2023 State Plan on Aging.

Responses to the survey are anonymous - DO NOT put your name, address, or phone number. Some questions require you to select multiple answers, or no answers at all (if the questions do not apply to you.)
