We all need a little help sometimes. Home care services help people age strong in the community, connecting them with in-home supports, adult day health programming, and/or assisted living services.

Sometimes all we need is a friend, the Senior Companion Program , pairs active seniors with older adults who aren’t as independent or mobile.

    Upcoming Community Meetings

    The Rhode Island Executive Office of Health and Human Services (EOHHS) is leading a committee to implement conflict-free case management (CFCM) and person-centered planning (PCP). The CFCM/PCP Committee is an interagency work group that comprises BHDDH, DHS, OHA, and EOHHS. Our goal is to create a person-centered planning and case management network that complies with federal and state requirements, is stakeholder informed, and leverages IT solutions to streamline services access, foster quality, and promote person-centered goals and outcomes. 

    We are inviting you to participate in a virtual community meeting to learn more about this project and the impact it will have on our Medicaid long-term care system. Please note that we plan on providing future webinars to stakeholders as we design and implement this program.  

    Please register for one of the Community Meetings using one of the links below. The presentation materials/content will be the same at each meeting. All meetings will be conducted via Teams.
